Support Services for Transition-Age Youth & Adults

We specialize in supporting transition-age youth (18–26) who are part of the Child & Family Specialty Plan (CFSP), including young adults aging out of foster care, individuals with mental health needs, and those navigating housing, independence, and stability. We also provide services for adults with intellectual and developmental disabilities (IDD) and individuals receiving Innovations Waiver or 1915(i) supports.

Peer Support Services

One-on-one support from someone with lived experience who understands mental health, recovery, and real-life challenges. We use Peer Support to help young adults and adults build hope, manage emotions, practice coping skills, navigate systems (courts, DSS, MCOs, housing), and make their own decisions about school, work, and relationships. Peer Support can look like coaching through tough days, talking through triggers, and practicing self-advocacy in real situations.

Community Living & Supports

Hands-on support in the home and community to help people stay housed, safe, and as independent as possible. We help with routines, cleaning, laundry, grocery shopping, cooking, transportation planning, and being a good tenant or roommate. Staff can go into the community for appointments, shopping, and activities, while coaching around boundaries, safety, and problem-solving in the moment.

In-Home Skill Building

Structured coaching in the skills needed to manage adult life. This includes personal care and hygiene routines, money management and budgeting, time management, organizing a room or apartment, communicating with landlords or employers, and handling day-to-day stress. Skill Building is goal-driven and focuses on practicing the same tasks repeatedly until the person can do them with less support.

Respite Services

Short-term support that gives caregivers, foster parents, or kinship placements a planned break while keeping the person safe and engaged. During Respite, we focus on positive activities, community outings when appropriate, and maintaining structure and routines so the youth or adult can return to their home or placement stable and supported. Respite helps prevent burnout and reduces the risk of placement disruption.

Supported Employment

Support with getting ready for work, finding a job, and keeping it. We help youth and adults explore what they're good at, create simple resumes, complete applications, practice interviews, and learn workplace expectations like showing up on time, communicating with supervisors, and handling conflict. We also coordinate with employers and training programs so work can be part of a longer-term plan for independence.

Individual Support / Community Networking

Support to build safe connections and meaningful roles in the community. This can include exploring clubs, faith communities, recreation centers, support groups, and peer groups; practicing social skills; and connecting with positive adults and natural supports. The goal is to reduce isolation, build confidence, and create a network of people and places that support long-term stability.

Request Support or Send a Referral

Clients, guardians, and providers use our online forms to start the process.

Intake Review

We review the information to confirm eligibility, safety, and service needs.

Intake Interview

We contact the client (and guardian if needed) to verify identity, gather details, and complete the Intake Interview Packet.

Consent & Planning

We complete consent forms and create a Person-Centered Plan (PCP) based on strengths, needs, and goals.

Staff Assignment

We match the client with qualified staff based on their service needs and schedule.

Services Begin

Services start in the community with ongoing communication between NSS, the client, guardian, and care team.
